5-Year Warranty

HERE AT STATEN ISLAND DENTAL CARE, we completely stand behind the quality of our work.

This certificate outlines the terms and conditions of the 5 year warranty on all restorative and cosmetic dental treatment. This does not apply to decay that occurs underneath a filling or crown.

Terms and Conditions:

Failure to completely uphold the following points, deem this warranty null and void.

1. Patient agrees to maintain a regular schedule of preventative hygiene care of no less than 4 annual visits at approximately 3 month intervals.
2. Patient agrees to make appointments that he/she can keep. Valid excused absence requires a Doctors note or an immediate family emergency. No excessive cancellations with Doctor or Hygienist.
3. Patient agrees to maintain a personal cash balance of zero.
4. Patient agrees to maintain home dental care as recommended by their hygienist.
5. Warranty is valid only on entire treatment plans. Should a phased approach be delivered, the warranty is valid only upon Doctors approval.
6. All terms and conditions are contained within this document; no other agreements apply unless placed in writing and attached to this document.
7. Warranty will be voided if for any reason you get into a traumatic accident or injury to your mouth.
